Accessing & Using Your Insurance Card

It is extremely important you have access to your insurance card at all times while in the United States. This page will detail how to receive your insurance card, and how to effectively use your insurance.

Accessing Your Insurance Card

Students do not automatically receive their insurance card when enrolled in the LewerMark plan. Students must access it themselves through the following process:

  • Visit lewermark.com/uca
  • Click on Student Login for ID Card
  • Your Student ID is your uCA student ID number, with a zero instead of the “B” (ex., 0012345678)
  • Your initial Password if your date of birth in mmddyyyy format (ex., 01081995)
  • After entering your ID & Password, you will be asked to select a new password
  • After changing your password, you will be taken to a Welcome Screen
  • Click on the small box in the upper-left-hand corner of the screen
  • Select Student ID Card.

Students should always have this card with them. It can be either printed or downloaded to phone. We recommend both.

No Insurance Card?

If you are not able to access your insurance card, please email international@uca.edu or call us at 501-450-3445.

If you must receive healthcare without your card, make sure to keep all prescriptions, bills, receipts, and any other paperwork you receive. You will be able to file for reimbursement for any payments that would have gone to insurance.

Where to Use Your Insurance

Please go to this section of the website to seesome of the local healthcare providers available to you, and when you should go to each. You can also use the Search for a Doctor database.

Using Your Insurance

Students will usually be asked for their insurance card upon their arrival to a healthcare provider. Please make sure you have your insurance card with you to give to them. Below are some pieces of information that may be helpful when arriving at a doctor’s office. If you are ever unsure of something, please give the person you are speaking with your insurance card. Please also read through our FAQ page.

Network Provider: Aetna

If asked who is your provider, the answer is Aetna. This is on the top-right hand corner of your insurance card. The doctor’s office will not recognize the name “LewerMark”. Aetna is one of the largest insurance providers in the United States. However, it may be the case that a particular doctor does not accept Aetna. Please know it will be more expensive for you to go there.

Co-Pays

Your co-pay is a small fee you must pay each time you visit the doctor’s office. LewerMark’s co-pay amounts are as follows:

  • UCA Student Health Clinic: Free
  • Office visits: $20
  • Hospital visits: $50
  • Emergency room visits: $100

Please be aware of how much the co-pay is.

Insurance ID Number

This is different from your student ID number. This is found on your insurance care, beginning with three 9s.

Claims Information

If a doctor or pharmacist has questions which you are unsure of, they can call the claims number listed on your card: 1-800-414-0596.

Insurance Billing

Even with insurance there can be a great deal of paperwork. It is your responsibility to keep up with everything. Failure to pay an outstanding bill can have serious repercussions.

Your default mailing address has been listed as the Office of International Engagement. Any insurance or healthcare related mail you receive will come to us, and we will email your UCA email account when it has arrived. Please be sure to promptly come and pick it up and please read it carefully.

Deductible

Your deductible is the amount of your bill you must pay before the insurance will begin paying. LewerMark’s yearly deductible is $100. This will reset each August 1 for every student, no matter when they were first enrolled in the insurance plan.

Explanation of Benefits

Your Explanation of Benefits (EOB) breakdowns what the provider is charging you, what insurance is paying, and what you are required to pay. Make sure to read them carefully.

Claims Questionnaire

Before paying, it is common for insurance companies to request more information on what you would like to have the company pay for (claim). LewerMark may mail you a claims questionnaire, seeking additional information about your injury or sickness. You will need to fill this out as quickly as possible.
