You may come by our office to complete the paperwork any time during business hours, without an appointment.
If you choose to print the paperwork, please complete it and bring it to our office during business hours. We are located in Student Health Center, Suite 327.  Completed paperwork must be hand delivered to the Counseling Center, or you may fax it to 501-450-3291.
