New Client Registration

WE DO NOT HAVE ONLINE APPOINTMENT SCHEDULING.

After registering as a new client, you will be contacted by a counselor to schedule your appointment. 

If you are currently seeing one of our counselors, please do not fill out the forms again; contact our office to schedule an appointment. 

Please contact our office as soon as possible if you cannot keep your scheduled appointment at (501) 450-3138.

Important Information

 

Consent Forms must be submitted to complete registration.

 

If you are submitting paperwork for Couples Counseling and your partner is NOT a student at UCA, they will need to use your Student ID.

 

You will be assigned to a counselor and should receive a call within 3-4 business days. If you would like to schedule with someone in the community, please see this list of providers that sometimes offer discounted rates for students.

 

Be sure to review the UCA Counseling Center Confidentiality Policy and  UCA Counseling Center Tele-Mental Health Information form.

 

This paperwork is not intended for Mental Health Emergencies. For emergencies after hours, contact the UCA PD at (501) 450-3111, dial 911, or go to your nearest emergency room. If you have an emergency during working hours please come by the Counseling Center (SHC 327) and you will be seen within a half hour.

 

If you are a UCA Faculty/Staff member and not currently a student, counseling is available through the Employee Assistance Program.
