In case of an incident or near miss in the laboratory, please fill out the following form and return it to the Chemical Hygiene Officer in Manion 303A. Incident Report Form
When filling up chemical waste containers, please attach a Hazardous Waste Identification label. When a chemical waste container is full, contact either the Chemical Hygiene Officer or the Lab Coordinator so it can be properly handled.
