OCCUPATIONAL HEALTH AND SAFETY PLAN FOR ANIMAL LAB PERSONNEL
All Animal Lab Personnel must abide by the University of Central Arkansas’ Occupational Health and Safety Plan. All faculty, students, and staff who work with or around lab animals on our campus must follow these instructions:
1. Carefully read the UCA Occupational Health for Animal Lab Personnel – Information Guide.
2. Complete the UCA Occupational Health for Animal Lab Personnel – Health Questionnaire.
3. Complete the UCA Occupational Health for Animal Lab Personnel – Documentation Form.
4. Turn in both the Health Questionnaire and the Documentation Form to Student Health Center.
5. Student Health will sign the Documentation Form for you and you will return the Documentation Form to the Research Compliance Office – Torreyson West 324.

All completed Documentation Forms will be securely kept on file with the corresponding approved IACUC protocol in the Research Compliance Office.
Completed Health Questionnaires will be part of your medical record with the Student Health Center. You must notify Student Health of any changes to your health information as soon as possible by submitting an updated questionnaire.
