The Community Care Clinic is now offering Patient-Reported Outcome Measurement Information System (PROMIS) screens with our care coordinator. This brief, 15-minute visit counts as a BeWell preventive visit and earns 25 points.
PROMIS screens provide insight into your physical, mental, and social well-being and can help connect you with supportive resources when needed.
Walk-ins are welcome on Tuesdays and Thursdays from 8:30 a.m. to 4:00 p.m.
