Are you looking for a particular form? We have a form page where all of our forms can be found.

2. Will my insurance or Medicaid/AR Kids pay for therapy and evaluations?

Our clinic does not accept third party payments. Your insurance may cover
speech therapy services, but we are not able to bill insurance companies. You may be able to be reimbursed by your insurance company for your out of pocket expenses. We can provide you with documentation for this if needed.

3. Do I need a referral?

A referral is not necessary for you to receive our services.

4. How do I make an appointment for an evaluation or therapy?

Contact Leslie George at 501-450-5776 or lgeorge@uca.edu about scheduling an
appointment for therapy or an evaluation.

5. Who provides therapy?

Speech therapy and evaluation services are provided by graduate students in the Department of Communication Sciences and Disorders under the supervision of our clinical faculty. Clinical faculty at the UCA SLHC are certified by the American Speech Language Hearing Association in speech language pathology or audiology and are licensed by the state of Arkansas.

6. Can I watch my child’s therapy session?

You are welcome to watch your child’s therapy session. Most of our therapy rooms have an observation deck attached, which allows you to watch and listen through a one-way mirror without your child knowing you are there.

7. Who do I call if I’m going to be late or absent?

If you are going to be late or absent for an evaluation, please contact our front desk at 501-450-3176. If you are going to be late or absent for a therapy session, please contact your assigned clinician. You and your clinician should exchange contact information during the first session. If you are not able to contact your clinician you may contact our front desk at 501-450-3176.

8. Where is the UCA Speech Language Hearing Center located?

The UCA SLHC has two locations. Our main building is located on the UCA campus on the northeast corner of Bruce Street and Donaghey Avenue, right across from Donaghey Hall. The Communication Sciences and Disorders Annex is located 2 blocks west of the main building. Travel west down Bruce Street, go straight through the traffic light at Bruce and Augusta Avenue, then take the next right onto Western Avenue.

9. Where do I park while I’m at the clinic?

While visiting our main building, turn into the small lot attached to our building and park in the blue lined client parking spaces located in the center row closest to the building. You will be given a parking pass to put on your dash when you arrive. While visiting our Annex, turn right into the parking lot after our sign and park in any yellow lined parking space. You will not need a parking pass to park at the Annex.
