Faculty can use this form to request experiential learning opportunities at the Community Care Clinic for students in their courses. These opportunities provide hands-on experiences that connect classroom learning with real-world community health needs, allowing students to apply their knowledge while supporting the clinic’s mission to serve the community. Faculty are encouraged to complete the form to help coordinate meaningful learning experiences for their students.
