How to Schedule Your Appointment
- Fill out a contact form here, give us a call, or send us an email about setting up a visit to our clinic.
- Registration forms will be sent to your email – please complete the form prior to your arrival.
- Bring a copy of your insurance card, along with the copay listed on the card, with you to your appointment.
- Check in at the front desk and see a provider!
Appointment Information
For your first appointment, please bring the following:
- Valid form of identification
- Insurance card and copay (if applicable)
- List of current medications (if applicable)
Please park in the designated client parking spots and display your parking pass in the dashboard of your car.
Accepted Insurance Providers
Most major insurances are accepted at our facility. This includes: United, Blue Cross / Blue Shield, Aetna, Anthem, Cigna, Medicare, Humana. For questions about your insurance eligibility or information about self-pay options, please call our clinic.
